Beth Israel Lahey Health Surgery Center Plymouth, LLC (the “Applicant”), with a principal place of business at 41 Resnik Road, Plymouth, MA 02360, intends to file a Notice of Determination of Need (“Application”) with the Massachusetts Department of Public Health for the creation of a freestanding ambulatory surgery center (“ASC”) to be located at 41 Resnik Road, Plymouth, MA 02360 (“Proposed Project”). The total value of the Proposed Project based on the maximum capital expenditure is $16,349,011. The Applicant does not anticipate any price or service impacts on the Applicant’s existing patient panel as a result of the Proposed Project.
